Something happened at HYROX Beijing in September 2026 that the HYROX community has been talking about ever since. During the Pro Women race, an athlete suffered what appears to have been severe bowel incontinence. She kept racing. She finished first. And because HYROX is an indoor competition built around a shared running course and shared equipment, what might otherwise have remained a deeply unfortunate and very private bodily malfunction became a question of hygiene, responsibility, sporting conduct and, inevitably, disqualification.

The reactions have been predictable. Some people admire the determination it took to continue. Others argue that she should have stopped immediately, particularly because other athletes still had to use the same course and equipment. There are reasonable arguments on both sides, and this article is not going to decide which one is right. In fact, the most interesting thing about Beijing may be that everybody started debating what should have happened after it happened. HYROX has detailed rules governing almost everything athletes do on the race floor. According to the official HYROX Singles Rulebook, spitting or clearing your nose onto the floor can result in a two-minute penalty or disqualification at the Race Director’s discretion. Littering is penalised. Athletes must use assigned equipment and lanes. Yet the current rulebook does not appear to contain a specific protocol for an athlete experiencing bowel or urinary incontinence during a race. Perhaps the question is therefore not simply whether one athlete should have stopped. Perhaps it is whether a global sport should already know what happens when a human body does something inconvenient.

Bodies Do Inconvenient Things

What happened in Beijing looks extraordinary. In endurance sport, the underlying problem really isn’t. One of the most famous examples happened in front of millions of people more than twenty years ago. During the 2005 London Marathon, Paula Radcliffe developed stomach cramps and stopped at the side of the road at around 21 miles because she desperately needed the toilet. She lost approximately 15 seconds, started running again and won the race in 2:17:42, more than five minutes ahead of second-placed Constantina Dita. World Athletics’ contemporary race report records both the roadside toilet stop and her eventual victory. It was Radcliffe’s third London Marathon win and, at the time, the fastest women’s-only marathon ever run.

Radcliffe knew toilets existed. She needed one there. That distinction matters because gastrointestinal distress is not an exotic failure of athletic preparation. Running and prolonged endurance exercise are well known for provoking gastrointestinal symptoms, including urgency and diarrhoea. Most runners who experience them will never find themselves in Radcliffe’s position, and almost nobody will experience something as extreme as what happened in Beijing, but athletes are still biological organisms. Digestion does not switch off when the timing chip switches on.

Neither does the bladder. This becomes particularly relevant when discussing female athletes. A 2026 umbrella review and meta-analysis of 32 studies covering 4,649 adult women athletes found an overall pooled prevalence of urinary incontinence of 39 percent, although prevalence varied substantially between studies and sports. Earlier research has similarly found that urinary incontinence is common among female athletes, particularly in activities involving repeated impact and increased intra-abdominal pressure. HYROX combines running with movements including burpee broad jumps, heavy sled work, lunges and wall balls. This does not mean that urinary leakage is inevitable in HYROX, nor does it mean that every woman needs special treatment. It means that designing a sporting event around an imaginary athlete whose bladder, bowel and reproductive system never interfere with competition is designing it around a body that does not always exist.

And Sometimes Women Get Their Period

There is an even more obvious example. Women menstruate. Sometimes a period arrives exactly when expected. Sometimes it does not. Sometimes an athlete starts bleeding halfway through an event she has spent months preparing for. This is neither a medical emergency nor a reason she should not compete, but it can become an event-design problem very quickly if there is no accessible toilet, no sanitary product and no realistic way of dealing with it without effectively leaving the competition.

This matters particularly because HYROX is remarkably successful at attracting women. Its own Sport Science Advisory Council has put increasing attention on female physiology and identifies menstrual cycles, contraception, pregnancy, postpartum recovery and menopause as areas that matter when researching female performance. HYROX describes inclusivity as being built into its DNA. That makes the practical question even more interesting: does the infrastructure surrounding the competition fully reflect the population actually competing in it?

At present, HYROX event information routinely includes toilets, changing areas, bag drops and other practical facilities, but I could find no publicly communicated global HYROX standard guaranteeing menstrual products such as tampons or pads at events. That does not mean that no HYROX venue provides them; individual venues may well do so. It means that an athlete cannot currently appear to rely on menstrual products being available simply because she is entering a HYROX event. If menstruation is a normal part of female athletic physiology, menstrual products are arguably less an amenity than basic sporting infrastructure.

Equal Toilets Are Not Equal Access

This leads to a deceptively simple question: how many toilets should there be? In Invisible Women: Exposing Data Bias in a World Designed for Men, Caroline Criado Perez uses public toilets as one of many examples of how apparently gender-neutral design can create unequal outcomes. Women take longer to use toilets for a range of perfectly mundane reasons: they generally need cubicles rather than urinals, clothing takes longer to remove, menstruation requires additional time, pregnancy can increase urinary frequency, and women are more likely to accompany children, older people or people who need assistance. Simply allocating the same amount of space to male and female toilets can therefore look equal while producing very unequal waiting times.

Anyone who has attended a large sporting event has seen the result. A short or nonexistent queue for the men’s toilets can exist metres away from a long line outside the women’s. At a HYROX event this becomes more than a minor inconvenience. Thousands of athletes arrive in waves. They are hydrating. Many have consumed caffeine or sports drinks. They are nervous. They may be trying to use the toilet shortly before entering the start tunnel. An athlete who has to join a long queue twenty minutes before her start does not experience the same access as an athlete who can walk straight into a urinal, even if the venue technically provided an equal number of square metres for each sex.

So perhaps genuine equality at a HYROX event would look deliberately unequal on the venue map. More female toilet capacity. Menstrual products available as standard. Appropriate disposal bins in every cubicle. Accessible toilets positioned where adaptive athletes can actually use them efficiently. And perhaps, most interestingly in light of Beijing, an emergency sanitary route that an athlete can access from the race itself without having to navigate halfway across an exhibition centre.

Fair Does Not Mean Removing Every Disadvantage

None of this means that HYROX should stop the clock every time somebody needs the toilet. That would create an entirely different fairness problem. Marathon runners do not receive a time-out because they need to pee. Paula Radcliffe’s 15-second roadside stop remained part of her 2:17:42. If an athlete loses 90 seconds because she needs to use a toilet during a HYROX, that may simply be part of racing.

But there is a significant difference between losing 90 seconds because you need the toilet and discovering that there is effectively no toilet you can access without leaving the authorised race environment. The same applies to menstruation. Nobody is suggesting that a woman should receive a competitive advantage because her period starts during the race. The question is whether she has a realistic way to deal with something her body is doing while remaining in the competition. Fairness does not require organisers to eliminate biology. It requires them to think about it.

HYROX Already Understands That Equal Is Not Always Fair

There is good reason to believe HYROX understands this principle because it already applies it elsewhere. Adaptive HYROX includes classifications for athletes with different permanent impairments, with modified requirements where appropriate. HYROX itself describes its adaptive classification system as an ongoing project as it works towards a more inclusive competition.

That is important. An adaptive athlete performing a modified movement is not being given an unfair advantage simply because another athlete performs the standard version. The modification exists so that the competition can test athletic performance rather than the arbitrary incompatibility between a particular body and a piece of equipment. Equality and fairness are not always the same thing.

The same principle can apply to infrastructure without changing a single repetition of the race. A wheelchair user may need a different route to an accessible toilet. A menstruating athlete may need immediate access to a tampon. A woman who has given birth may experience stress urinary incontinence. An athlete with gastrointestinal disease may suddenly need a toilet. Another athlete may simply have eaten something that disagreed with them that morning. These people do not necessarily need different sporting standards. Sometimes they simply need an event designed for actual human bodies.

Then There Is Another Kind of Fairness

All of this becomes even more interesting when we separate access fairness from competitive fairness. HYROX describes itself as a global fitness race in which every event follows the same fundamental format, allowing athletes to compare performances across events. Of course, that does not mean every HYROX can have identical conditions. It would be impossible.

The first HYROX Tenerife in September 2026 is a useful example. Athletes described extremely hot conditions inside the venue, with reports of high temperatures, humidity, insufficient cooling and competitors struggling badly with the heat. Those reports deserve a separate discussion about athlete safety and venue selection, but heat itself raises an interesting fairness distinction. Tenerife being hotter than Hamburg is not automatically unfair. A marathon can be cold one year and hot the next. A trail race can be dry in May and ankle-deep in mud after a week of rain. A road race can have a headwind. Environmental conditions are part of sport, and athletes competing at the same place and time are broadly facing the same problem.

Equipment is different. The HYROX rulebook requires athletes to use the assigned equipment and lanes. If one sled lane therefore produces substantially more resistance than another, athletes are no longer simply facing a difficult competition. They are facing different competitions. One athlete can be stronger, fitter and better prepared than another and still lose time because the randomly assigned lane happens to move differently.

Hard conditions are part of sport. Unequal conditions are a fairness problem.

A hot HYROX can still be a fair HYROX if everyone competing against each other faces essentially the same heat. A brutally heavy sled can still be fair if every athlete has to move an equivalently brutal sled. What cannot easily be defended as competitive variation is one athlete pushing a sled that moves normally while another athlete of the same division and competition encounters substantially greater resistance because of the carpet, sled or lane beneath it. At that point, the result is no longer determined exclusively by the athletes.

Fairness Has More Than One Meaning

Perhaps this is why the Beijing incident is worth discussing beyond the obvious shock value. It exposes several different meanings of fairness at once. Competitive fairness asks whether athletes are being given an equivalent sporting challenge: comparable equipment, consistent judging, correct distances and properly functioning sled lanes. Access fairness asks whether athletes with different physical needs can realistically participate: accessible venues, adaptive divisions, appropriate toilets and infrastructure. Physiological fairness asks whether event design acknowledges predictable realities of the bodies taking part: menstruation, pelvic-floor dysfunction, gastrointestinal distress and other biological events that are neither misconduct nor particularly unusual.

None of these require HYROX to make racing comfortable. It is supposed to be uncomfortable. That is rather the point. They require something more subtle: separating discomfort that belongs to the sporting challenge from disadvantage created unnecessarily by the infrastructure around it.

So Should the Athlete in Beijing Have Been Disqualified?

I still don’t know.

There is a legitimate hygiene argument for stopping an athlete whose bodily fluids may contaminate a shared indoor course and equipment that dozens of other people will immediately use. There is also an important distinction between an involuntary bodily event and deliberate misconduct, and between punishing an athlete for becoming ill and protecting everybody else on the race floor. The current rules make the question even more complicated: HYROX explicitly penalises spitting and clearing the nose onto the floor, yet does not appear to provide a specific rule for the far more serious sanitary situation that occurred in Beijing.

Reasonable people can disagree about what the Race Director should have done. What seems much harder to disagree with is that the Race Director should not have to invent the answer while the athlete is already moving towards the next station.

A mature global sport can have a protocol. An athlete experiences significant bowel or urinary incontinence. A marshal knows what to do. There is an identified sanitary route. There is a procedure for cleaning or replacing contaminated equipment. There is a predetermined rule governing whether the athlete can continue, receives a penalty or must withdraw. Athletes know that rule before they start. Officials know how to apply it. Other competitors are protected. Nobody has to decide in the middle of a viral moment what fairness suddenly means.

The Athlete Should Be the Variable

HYROX has grown because its concept is brilliantly simple. Eight one-kilometre runs. Eight workout stations. The same fundamental challenge around the world. It gives ordinary athletes something unusually powerful: the ability to compare themselves not only with the person in the next lane, but with themselves in another city months later.

Perfect standardisation will never exist. Tenerife will sometimes be hotter than Hamburg. Athletes will get sick. Women will menstruate. Pelvic floors will occasionally fail under load. Wheelchair users will require different infrastructure. Stomachs will revolt at spectacularly inconvenient moments. None of that makes sport unfair.

But the equipment should work. The rules should anticipate foreseeable situations. The toilets should reflect the people who actually need to use them. Accessibility should mean more than being allowed through the front door. And when two athletes stand on the same start line, the differences in their results should come as far as possible from their fitness, strength, pacing, preparation and decisions rather than from the sled lane, toilet queue or piece of infrastructure they happened to encounter.

Fair does not mean identical. Fair means that the athlete, not the infrastructure, should make the difference.
